Since you are a minor, parental consent is required in order to participate in an SFL event. To do so, please open one of the three waiver forms available below and follow the four steps.
1. Print the form.
2. Have the form signed by a parent or guardian.
3. Scan the form.
4. Send us the file by email.
If you do not have a scanner, please bring the signed form on the day of the event.
